Request for Proposal for Trainer for Increasing Partnership Through the Use of Data and Information

The Department of Health (MDH) requests proposals to provide training and other learning opportunities to Health Care Homes (HCH) Learning Community grantees. The training and learning opportunities will focus on increasing the capacity of partnerships between primary care, behavioral health and local public health to use and share data to improve the health of the shared population.
