This form, prescribed by the Commissioner of the Department of Energy and Environmental Protection, is to be recorded on the land records pursuant to section 22a-409(a) of the Connecticut General Statutes (CGS). Effective October 1, 2007, section 22a-409(a) CGS requires that an owner of property containing a high hazard dam or significant hazard dam, record on the land records in the municipality where the dam is located, a document that identifies the existence of the dam and whether the dam is categorized as a high hazard dam or a significant hazard dam.
