License Renewal Tuition Refund Affidavit

State of Louisiana Board of Regents form for the affiants to state that the named Louisiana Proprietary School has refunded to the appropriate lending institution all student tuition refunds that became payable, according to state and federal law and regulations, prior to the date of this affidavit.
