Colorado Commission Application

The insurance company is authorized to insure against loss from explosion of boilers in Colorado (Insurance Company Name) and, pursuant to the Boiler and Pressure Vessel Statutes, requests that the Division of Oil and Public Safety issue a Colorado Commission as Special Boiler Inspector to the following inspectors employed by said insurance company.
