Authorization for Court Activity Record Information Check (CARI)

Allows the applicant to authorize the Juvenile Court to conduct a court activity record information check to determine whether they have a criminal record. They understand that the court activity record information check is intended for use by the Court in conjunction with the applicant's petition for guardianship of the named minor.
