Alcoholic Beverage Permit Application

Form required for any person that sells (directly or indirectly), manufactures, or allows the consumption of alcohol on the premises of any establishment located in the City of Lafayette and the unincorporated area of Lafayette Parish to first obtain a local and state Alcoholic Beverage Permit.
