State of Louisiana Department of Insurance form for the Designated Individual Producer to depose and say that the Agency Producer Applicant will have a principal place of business at the listed location, that the Agency Producer Applicant has never had its insurance producer license suspended, revoked or denied by any jurisdiction, and that the Designated Individual Producer has never had their insurance producer license suspended, revoked, or denied by any jurisdiction.
